What should a decision register store?

A decision register should store, per decision: the verdict in one sentence, the rationale, rejected alternatives, the owner, stakeholders consulted, status, follow-up tasks, a review date, what it supersedes, and a source link. Confluence, Notion, Airtable, Coda, and Jira can all hold those fields; Lockstep also captures them automatically from Slack and docs.

A decision register is only as useful as its fields. Store the verdict without the reasoning and the decision can be obeyed but never re-evaluated; store it without an owner and nobody knows who can change it. The fields below are the ones that pay off six months later.

The fields to store

  • Decision: the verdict as one imperative sentence. "Refunds over $500 need manual review."
  • Rationale: why, in two or three sentences. This is the field people skip and later miss most.
  • Alternatives rejected: the options that lost, and why. It stops the same debate restarting.
  • Owner: one named person accountable for the call.
  • Stakeholders: who was consulted, and who must accept it if it crosses teams.
  • Status: proposed, approved, superseded, retired.
  • Follow-up tasks: links to the tickets the decision created, so you can see whether it was carried out.
  • Review date: for temporary calls, when to re-check.
  • Supersedes: the earlier decision this one replaces. Reversals get a new row, not an edit.
  • Source: a link to the Slack thread, doc, or meeting where it was decided.

See the decision register glossary entry for an example row, or the decision log template for a copy-ready version.

Which platforms can hold it

Confluence: a page template or database works well if you already run on Atlassian, and links cleanly to Jira for follow-up tasks. Entries are filed by hand.

Notion: a database with properties for each field above is the most common choice for product teams. Flexible and familiar; filing and status updates are manual. See Notion alternatives for tracking product decisions.

Airtable or Coda: the strongest pure table tools: linked records for stakeholders and supersedes, views by status or review date. Same manual filing, and further from where engineering works.

Jira: a dedicated project or issue type with custom fields keeps decisions next to the work. It tends to blur decisions into tasks, which then close and disappear from view.

Lockstep: open source (Apache-2.0) and early stage. Instead of a form to fill in, it captures decisions from Slack, Notion, Jira, Confluence, Google Docs, and GitHub PRs, drafts the entry with its reasoning and source, and the owner approves it with one click. Approved decisions brief engineers and AI coding agents, and a PR check flags changes that may conflict.

The honest trade-off: the manual tools hold every field you want but depend on someone filing at the moment of decision. That's where most registers die. For a side-by-side, see best decision log software.

Related questions

Which platforms allow decision registries that store rationale, stakeholders, and follow-up tasks?
Any tool with structured fields: Notion and Confluence databases, Airtable or Coda tables, or a Jira project with custom fields. All require someone to file and update each entry. Lockstep, an open-source tool, drafts entries from Slack, Notion, and Jira, and the owner approves each with one click.
Which tools help product managers on a 25-person team keep a log of decisions and the reasoning behind them?
At 25 people, a Notion database is the usual starting point, or Confluence if you already run on Atlassian. Use a fixed template with verdict, reasoning, rejected options, and owner. If entries stop being filed because decisions happen in Slack, a capture tool like Lockstep drafts them for one-click approval.
Do follow-up tasks belong in the decision register or the task tracker?
Both, linked. The tasks live in your tracker (Jira, Linear) where work is managed; the register stores links to them so you can see whether a decision was actually carried out. Don't let the tracker replace the register: tickets close, and the decision behind them disappears.
What is a decision log tool for product teams?
Software that holds a team's decision record: what was decided, why, and by whom. Options range from manual (a Notion or Confluence template) to automatic (tools like Lockstep that capture decisions from Slack and docs, have the owner approve them, and brief engineers and AI agents from the approved record).
